Complete an Education Benefits Form online (recommended). Online applications go directly to the Food & Nutrition Office. This prevents them from getting lost or misplaced.

Complete a paper application using the forms sent home in student opening packets! You may also print off a paper application - see printable application.
Blue or Black Ink must be used on paper applications.

We cannot accept applications filled out in pencil. Below are instructions and answers to frequently asked questions. You may submit applications at any time during the school year. The school year starts July 1 each year.

the attention of Karen Hagerman/F & N Services. 

Applications cannot be processed if information is missing or incomplete.

If you are NEW to the district and received Meal Assistance from your previous school, you MUST fill out a NEW application - benefits do NOT transfer automatically!

Benefits for applications do not take effect until the Office of Food & Nutrition has processed the application and notified you of your benefit status. Any meals purchased before this notification are the responsibility of the parent/guardian. You will be notified by email if one is provided, and a paper copy will be mailed; please make sure your address is correct at the school your student attends.
